NEW CLIENT FORMS

New Specialty Patient Intake Form
This is for new clients whose pets are meeting with one of our doctors for the first time. The information you provide will facilitate your pet's inclusion into our computer system and ensure timely communication with your primary care veterinarian.

Emergency Patient Intake Form
Fill this form out to aid our emergency staff in making decisions in regard to your pet's medical treatment.

Behavior Consultation Questionnaire (Dr. Leslie Cooper)
Please complete this questionnaire before your pet's appointment with Dr. Cooper, and either email it to sfvs@vca.com or fax it to (415) 401-9201 in advance of the appointment.

Oncology Patient Update Form
To be able to provide the very best care to our oncology patients, we ask that you please complete this form to tell us briefly how your pet has been feeling and to inform us of any medication needs.

Drop-Off Intake Form
This is for clients who are dropping off their pet for a scheduled procedure, such as MRI or surgery. The information you provide will facilitate your pet’s timely and accurate care, help us communicate with you about your pet, and see to your pet’s pharmacy needs.

Chemotherapy Patient Update Form
This is for clients who are dropping their pet off for chemotherapy. This information will be used to make important decisions about your pet's treatment. Inclusion of contact information and pharmacy needs will allow us to discharge your pet, pills in paw, as soon as they are ready.

Oncology Patient Update Form

Drop-Off Intake Form: DENTAL
This form is for clients who are dropping off their pet for a dental exam and/or procedure. We appreciate your taking the time to fill out this form. It helps to ensure we provide the best possible care for your companion and better service for you.

Tech Time Intake Form
Use this intake form when your pet has an appointment to see a member of our nursing staff on behalf of a doctor. Examples for tech time appointments would be: incision checks, splint checks, medication administrations, injections, and other instances where you do not have an actual appointment time to meet with your pet's clinician.

Emergency Care

In an Emergency ...

1. Contact your general veterinarian's office. They may be more closely located and available to assist you.

2. If it is after hours or VCA SFVS is more conveniently located, call us at (415) 401-9200 and provide the client service representative with:

  • Your name and your pet's name
  • Your location and estimated time of arrival
  • Your pet's emergency

3. Get support to safely transport your pet to VCA SFVS. If you have medical records related to your pet’s emergency, please bring those with you.

  • Complete intensive care unit (ICU) laboratory
  • Endoscopic foreign body removal
  • Full range of blood products
  • Oxygen therapy
  • 24-hour ICU monitoring by veterinary nurses trained in critical care, surgery and internal medicine
