Referral Forms Overview

To have your patient evaluated by one of our veterinary team members, please ask your client (your patient's guardian) to contact one of our administrative staff at (212) 924-3311 to schedule an appointment with one of our doctors.

In order to help our veterinary staff accurately assess your patient's current health status, we ask that prior to referral that you please complete our FAVS Referral Form located at the link below and fax it to our hospital at fax (212) 924-7228. Pertinent medical records may also be faxed along with the referral form or you may have the owner bring along with them the medical records, radiographs and laboratory test results. If you need assistance, have questions or wish to discuss your patient's case prior to referral, please call our hospital and a member of our staff will be happy to assist you. Our doctors welcome your call to discuss the referral case at any time.
